This two visit evaluation provides an initial one-hour evaluation visit to define areas of concern.  Initial recommendations will be made.  Appropriate diagnostic testing will be cooridinated.  A one-hour follow up visit to review testing and make additional specific recommendations completes this plan.

Additional visits may be recommended and may be obtained at our typical per visit charges.

Total cost - $750.00
